mcpApprovalRequests

Operator agents' held tool calls in the current tenant, newest first: the open ones (pending, approved, executing) unless status names others, at most limit (0-100). An App or a person sees the holds it made; a person with MANAGE_MARKETPLACE, who may decide them, sees the whole queue.
